1. Fill out the online application, complete and sign.

2. Attach the following documents:

Three months previous processing or bank statements (if available),

a voided check or bank letter showing a banking relationship (where funds will be deposited),

a clear copy of the owner's driver license or passport.

You can complete your application using the same name on your bank account. The patient's credit card statements will show the following- "POS Transaction".

3. E-sign completed application and supporting documentation and complete the underwriting process.
